2025-2026 Course Request Forms

Students entering Bernard Middle School for the 2025-2026 school year must complete an online Course Request Form. The sixth grade, seventh grade and eighth grade forms are available now.

Incoming 5th grade families and Current sixth and seventh grade students  are asked to please complete the 2025-2026 Bernard Online Course Request form by Thursday, February 20th.
