Course Request Forms
Students entering Bernard Middle School for the 2026-2027 school year must complete an online Course Request Form. The sixth grade, seventh grade and eighth grade forms are available now.
Incoming 5th grade families and Current sixth and seventh grade students are asked to please complete the 2026-2027 Bernard Online Course Request form by Friday, February 13.
